EXERCISE IN TYPE 1 DIABETES - BY DR. SHUCHY CHUG
Exercise is known to be good for children and teens with type 1 diabetes. However, whenever children with type 1 diabetes exercise or take part in other physical activities, such as sports, they run the risk of having a hypoglycemic episode (when blood glucose levels are too low) or, if they snack too much before the activity, a hyperglycemic episode (when blood glucose levels are too high). However, the benefits of exercise usually outweigh the risks and knowledge of the interactions of insulin and exercise will minimize any uncertainties.
Basic guidelines to minimize risks of hyper or hypoglycemia related to exercise:
- Metabolic control before physical activity.
- Avoid physical activity if fasting glucose levels are >250 mg/dl and ketosis is present, and use caution if glucose levels are >300 mg/dl and no ketosis is present.
- Ingest added carbohydrate if glucose levels are <100 mg/dl.
- Blood glucose monitoring before and after physical activity
- Identify when changes in insulin or food intake are necessary.
- Learn the glycemic response to different physical activity conditions

- Consume added carbohydrate as needed to avoid hypoglycemia.
- Carbohydrate-based foods should be readily available during and after physical activity.
- A standard recommendation for diabetic patients, as for nondiabetic individuals, is that physical activity includes a proper warm-up and cool-down period. Proper footwear is essential. Individuals must be taught to monitor closely for blisters and other potential damage to their feet, both before and after physical activity.
- A diabetes identification bracelet or shoe tag should be clearly visible when exercising. Proper hydration is also essential, as dehydration can affect blood glucose levels and heart function adversely.

There is a common myth that diabetics cannot eat rice. That's not true. The caloric value of rice is very close to other cereals like wheat but rice has a tendency to increase the blood sugars very quickly due to its high glycemic index. Therefore rice should always be combined with some other food, preferably lots of vegetables or dals, as they have a high fiber content, which prevents a rapid rise in blood sugar levels. Brown rice or unpolished rice is better choice as compared to white polished rice as they have more fiber.

Have you ever seen a small baby laughing himself while cycling on his bed, just think of the moment why the baby is so happy in this act, what pleasure he is getting in it? It's pleasure of nature his hormones are getting streamed in his blood with little bit of exercise and over all he is away from all worries and sorrows of this materialistic world. (That is what you are getting during morning walk in a natural atmosphere, if you can really feel the sound of nature)
Now get ready to go back in your childhood during your exercise program, feel the sound, feel the colors, feel the flow of wind and get your mind away from stress. Then you can feel the real pleasure of life.
